Responsibilities
Provide clinical and medical oversight for participant care
Lead and collaborate with an interdisciplinary care team
Oversee clinical outcomes and Quality Assurance Performance Improvement (QAPI) initiatives
Develop and maintain medical policies, protocols, and standards
Supervise Primary Care Providers and Advanced Practice Providers
Participate in participant assessments, care planning, and family conferences
Qualification
Required
Active and unrestricted Massachusetts physician license
Board Certification in Internal Medicine or Family Medicine (Geriatrics preferred)
DEA and CDS certification
Prior supervisory experience required
Minimum 1 year of experience working with frail or elderly populations
Strong skills in physical assessment and chronic disease management
Interest in working within an innovative, community-based care model
