The positions focus on high-quality outpatient care with a supportive, team-based environment and an emphasis on continuity, prevention, and chronic disease management for adult patients.

Responsibilities
Practice locations across Southbury, Naugatuck, and Shelton, CT (final site(s) to be aligned with candidate preference and practice needs)
Established patient panels with strong referral network and community presence
Monday–Friday schedule with a focus on work-life balance (minimal call, details to be discussed)
Multi-disciplinary team including APPs, nursing, and care coordination support
Qualification
Required
MD or DO degree with completion of an accredited Internal Medicine or Family Medicine residency
Board Certified or Board Eligible in Internal Medicine or Family Medicine
Eligible for Connecticut medical license
Commitment to compassionate, evidence-based primary care and collaborative practice
