Responsibilities

Provide comprehensive primary care to adult patients, including preventive care, acute visits, and chronic disease management
Diagnose and manage common internal medicine conditions such as diabetes, hypertension, hyperlipidemia, heart disease, COPD/asthma, kidney disease, thyroid disorders, and mental health concerns
Perform and interpret clinical assessments, develop treatment plans, and coordinate longitudinal care
Order, interpret, and follow up on diagnostic testing (labs, imaging, EKGs, pulmonary function tests as applicable)
Provide health maintenance services including annual physicals, screenings, immunizations, and lifestyle counseling
Coordinate referrals and collaborate with specialists, hospital teams, and ancillary services to ensure continuity and timely follow-up
Document encounters accurately and efficiently in the electronic health record (EHR)
Participate in quality improvement initiatives and adhere to clinical guidelines, safety standards, and regulatory requirements
Support patient education and shared decision-making to improve understanding, adherence, and self-management

Required

MD or DO from an accredited medical school
Completion of an ACGME/AOA-accredited Internal Medicine residency
Board Certified or Board Eligible in Internal Medicine
Active medical license in the practicing state (or eligibility to obtain)
DEA registration (or eligibility to obtain)
Strong communication skills and a commitment to compassionate, adult-focused care

Preferred

Experience in outpatient internal medicine, hospitalist work, or integrated health systems
Comfort managing medically complex patients and multiple comorbidities
Experience with value-based care, population health, and quality metrics
Interest in geriatrics, preventive medicine, or chronic care program development
Bilingual abilities aligned to patient population needs (e.g., Spanish)
