Responsibilities
Provides outreach to members via phone to support care plan next steps, community or health plan resources, scheduling questions, and ongoing education for members and providers
Connects members to health plan and community resources to ensure high-quality customer service
Applies working knowledge of assigned health plan activities and resources
Serves as front-line support for member and provider inquiries, explaining care plan procedures and protocols
Supports member onboarding and administrative duties including sending welcome letters, correspondence, and educational materials
Documents and maintains non-clinical member records in compliance with state and regulatory requirements and provides records to providers as needed
Refers members to local benefits and resources to address Social Determinants of Health needs
Performs other duties as assigned

Qualification
Required
High School diploma or GED
1 to 2 years of related experience
