As a Care Coach, you will support adult patients by conducting health assessments and coordinating care to ensure they receive the right healthcare services.

Responsibilities

Support adult patients, many of whom are elderly or chronically ill by conducting health assessments, coordinating care, and guiding them through their healthcare journey
Work in collaboration with members of the healthcare team to ensure patients understand their health benefits and receive the right care at the right time

Required

Bachelor's or associate degree in any discipline
Minimum of 2 years in direct adult patient care in similar settings
Proficiency in Microsoft Word, Excel, Outlook, and electronic records systems
Strong interpersonal and communication skills
Ability to build trust with patients, colleagues, and healthcare stakeholders
Critical thinking and problem-solving mindset

Preferred

Experience with Medicare/Medicaid recipients
Familiarity with health assessments and benefit coordination
Background working with elderly or chronically ill patients
Experience in BPO healthcare
