They are seeking a full-time Wraparound Facilitator to support at-risk youth by leading the Child and Family Team and coordinating activities to ensure youth stay at home, in school, and out of trouble.

Responsibilities

Build and lead the Child and Family Team (CFT) to support youth on a mission to stay 'at home, in school, and out of trouble.'

Coordinate team activities to achieve this goal while providing direct support and education to families and team members

Guide the family in accomplishing objectives outlined in the Family Plan and ensure accurate documentation in the UNCG database

Embody the Wraparound philosophy of 'Whatever it takes' to meet the unique needs of youth and families, driving meaningful, lasting success

Qualification
