Patient Coordinator

Giving Home Health Care is a leading provider of personalized, in-home care for individuals impacted by health conditions related to their work in nuclear facilities. The Patient Coordinator will support patients by facilitating communication between them and healthcare providers, conducting home visits, and ensuring resource delivery and patient well-being.

Responsibilities

Will receive the patients from Patient Development department through a warm handoff process
Attend the patients’ doctor visits to provide support, address concerns, and facilitate communication between patients and healthcare providers
Conduct home visits to assess patients’ needs, deliver resources, and provide additional assistance as required
Conduct an initial set up home visit for patients newly approved for home health care. Provide information on the program, as well as facilitate caregiver onboarding
Deliver comprehensive patient binders to patients’ homes containing plan of care and training information
Serve as the main point of contact for patients, ensuring they have a dedicated resource for all inquiries, concerns, and updates regarding their care
Make courtesy calls to patients to check on their well-being, address immediate concerns, and maintain open lines of communication between the patient and the Giving Home Health Care clinical team
Conduct milestone calls to track patient progress, update them timely regarding their care, approvals, caregiver status, and any other updates to their care

Required

Strong work ethic and relationship building skills
Self-motivated with the ability to manage time appropriately
Strong communication skills inside of the office and outside
Experience with MS Office products (Outlook, Word, Excel, etc.)
Minimum associate level education preferred. (High school diploma or equivalent is required.)
Minimum 1-3 year of experience in a Care Team or Patient Navigation role
Must have valid driver's license and your own transportation
Dependable transportation to be able to travel locally between locations including patient homes, physician offices and Giving offices
