Responsibilities
Coordinates the referral development system designed to facilitate referrals to the Unit
Conducts thorough pre-admission assessments of rehab candidates from phone inquiries, faxed referrals and onsite assessments when appropriate. Makes acceptance recommendation to Medical Director and/or Director of Rehabilitation Services
Collaborates with in-house staff to offer rehabilitation services to patients at NNMC who qualify. Coordinates with staff to transfer patients promptly to the Rehabilitation unit to insure optimal patient care
Obtains insurance verification and authorization to insure that the rehabilitation services are reimbursable
Qualification
Required
License per related field required
Preferred
Bachelor's degree in related field preferred
Minimum of 2 years of clinical rehabilitation experience preferred
Experience or knowledge of Medicare Rehabilitation regulations preferred
Experience in Case Management preferred
