Client Services Field Specialist
24 Hour Home Care is part of the TEAM Services Group portfolio of companies, and they are seeking a dependable, compassionate, and detail-oriented Client Services Field Specialist/Regional Coordinator. This role involves ensuring visit compliance, supporting family providers, and assisting with policy rollouts and quality standards to deliver high-quality, compliant care across Arizona.

Responsibilities
Conduct and document timely 90-day in-home visits to ensure compliance with Arizona DDD and HCBS requirements
Maintain accurate, up-to-date case documentation and submit visit reports on time
Monitor provider compliance with agency policies and support the rollout of updated procedures
Serve as a liaison between the Regional Director, client families, and providers to ensure service consistency
Assist with regional audits, training events, and special initiatives
Provide in-person support and training to new and existing family providers
Support relationship-building and collaboration between client families and care teams
Qualification
Required
Level I Fingerprint Clearance Card
Fluency in Spanish is required
High school diploma or equivalent required
1+ year experience in HCBS, social services, or administrative field support
Familiarity with Arizona Division of Developmental Disabilities (DDD) and HCBS regulations
Valid Arizona driver's license and reliable daily transportation
Comfortably conducting daily home visits and working in various community settings throughout the entire Phoenix valley
