Regional Systems Liaison Contractor (Current – June 2026)

Full Circle Health Network is an integrated network of nonprofit providers delivering coordinated, community-based services to vulnerable children, individuals, and families across California. The Regional Systems Liaison Contractor will build and nurture relationships with systems related to child welfare and justice, educate and increase participation in Medi-Cal programs, and represent Full Circle in collaboration with public departments and social service agencies.

Government Administration

Responsibilities

Develop and maintain strong partnerships with social service agencies across the State, particularly those involved in child welfare and the justice system
Engage with system leaders to understand their needs, challenges, and goals, providing tailored support to increase their engagement and referral of members to CalAIM services
Coordinate training and technical assistance for Enhanced Care Management (ECM), Community Health Worker (CHW) Services, and Community Supports (CS)
Collaborate with designated FCHN points of contact to align support services with the regional needs of each system
Serve as a liaison between local systems and FCHN, advocating for the needs and perspectives of providers within larger policy and program discussions
Conduct outreach to identify and engage new systems-based partners, particularly those serving children and families
Develop a CalAIM toolkit for key regional stakeholders, including Child Welfare, Justice, Specialty Mental Health, and Housing departments, to support the whole-person, integrated care model
Meet with local CalAIM teams, providers, and other key stakeholders as needed
Attend outreach events across the State as needed
Provide education on FCHN and CalAIM services to systems serving children and youth in the assigned region
Track and report on the progress of partnership initiatives, ensuring alignment with grant objectives and FCHN’s mission

Required

Bachelor's degree in Social Work, Public Health, Community Development, or a related field
Minimum of 3 years of experience working with child welfare, justice-involved, or behavioral health program coordination, or in a partnership development role
Demonstrated experience supporting organizations focused on transitional-aged youth, foster care, or system-involved populations
Familiarity with Medi-Cal programs, ECM, CHW Services, and Community Supports
Strong interpersonal and communication skills, with the ability to build and maintain effective partnerships
Knowledge of the State social services landscape, particularly in the context of child welfare and justice-involved services
Ability to navigate complex administrative and regulatory environments
Strong organizational skills with the ability to manage multiple projects and priorities
Ability to travel within the region to meet with partners and stakeholders
Commitment to FCHN's mission and values, particularly in supporting vulnerable populations and promoting equity
