Responsibilities

Assists patient with all aspects of personal activities of daily living and oral hygiene care while promoting resident self-direction and personal decision-making while protecting resident's health and safety
Safely assists patients with transfers to bed, chair and commode, as well as ambulation; use of walker, cane
Safely assists bed-bound patients with appropriate range of motion exercises and required turning and positioning. Demonstrates safe and effective use of hoyer lift as instructed by the RN
Performs shift to shift report (rounding); including patient observation, repositioning as well as wet checks
Responsible for learning and utilizing technology made available by the agency to increase efficiency, communication and performance
Performs all aspects of catheter care according to policy and procedure
Prepares equipment for return to vendor and places in appropriate storage areas. Stocks supplies according to inventory and alerts appropriate staff when supplies are low
Applies ostomy bags, and simple dressings as instructed and demonstrated by the RN
Properly prepares rooms for new admissions; i.e. zeroing bed, oxygen setup; suction setup as applicable
Provides skin care at the direction of the RN in order to prevent skin breakdown and report observations to RN
Assists patients at meal times including but not limited to setting up, feeding, and tray distribution as assigned and defined by Aide Care Plan
Observes the patients physical and emotional status and reports on a timely basis any changes in the patient's condition or family situation
Assists with post-mortem care as instructed by the RN
Accurately completes all aspects of documentation before leave of shift and reports off to incoming RN and Hospice Aide
Safely changes occupied or unoccupied bed. Washes patient's personal laundry and bed linens when needed
Observes and participates in the Emergency Preparedness/Disaster plans that are required by the facility. Maintains the appropriate infection control practices
Actively participates in maintenance of building/grounds and completes all assigned tasks in a timely manner. This includes but not limited to maintaining accurate temperature logs/cleaning schedules
Maintains the appropriate infection control practices
Demonstrates proper mouth and oral hygiene care
Answers call bells promptly and follow through with patient requests
Participates in all required in service education programs and meetings
Demonstrates a commitment to own work schedule by adhering to attendance standards. Provides proper notification in advance for absence and tardiness
Adheres to employment policies in accordance with the Employee Handbook
Demonstrates compliance with the dress code policy by appearing well groomed, wearing ID badge while on duty and maintaining a professional appearance
Performs other duties as assigned

Required

Knowledge of skills required to provide excellent care in accordance with the role of Certified Home Health Aide (CHHA) or Certified Nurse Aide (CNA) in a hospice environment
Excellent interpersonal skills for interacting with patients and families, nurses and employees, and patients and families
Ability to comprehend the spoken and written English language, to communicate with clarity both verbally and in writing
Visual acuity sufficient to read a thermometer and auditory acuity sufficient to hear and comprehend normal speaking tones, and to hear call bells with or without assistive devices
Well organized to manage time, prepare care-related documentation
Ability to complete a minimum of 12 hours of in-service education per year
High School diploma or equivalent
Must possess current Home Health Aide Certification by the New Jersey Board of Nursing, or a Certified Nurse Aide approved by the New Jersey State Department of Health and Senior Services
