Responsibilities

Performs telephonic patient coordination, discharge follow-up with physicians and patients and disease management programs

Coordinates with other clinics and hospitals assisting with physician consultation, transfer and coordination of patient ground/air transport

Collaborates frequently with Emergency Room (ER), and Post-Anesthesia Care Unit (PACU) and bed boards to keep abreast of possible admissions/transfers

Collaborates with the Bed Assignment/Staffing Group, Patient Placement Coordinator or Administrative Supervisor per shift to determine census and acuity

Collaborates to resolve bed availability challenges

Receives the admission orders and ensures that orders are sent to the appropriate nursing unit

Documents coordinator and patient interactions in electronic or hardcopy chart in a manner consistent with hospital standards

Creates reports from patient data such as volumes and outcomes

Performs other job-related duties as assigned
