Direct Support Coordinator
Adjoin is a Non-Profit organization focused on building a future where all people can belong in their communities. They are seeking a Direct Support Coordinator to empower individuals with intellectual and developmental disabilities by fostering independence and facilitating their inclusion in community activities.

Responsibilities
Maintain current knowledge of client’s needs
Maintain positive communications and relationships with his/her supervisor, clients, landlords, employers, involved members of families, Regional Center, D.O.R. and other individuals in the community
Advance client’s decision-making, productivity and participation in a range of daily activities
Oversee client activities and environment throughout shift to ensure the fulfillment of health and safety needs
Facilitate the client’s inclusion in positive community experiences
Transport clients to appointments, community activities, or other locations as needed, ensuring safety and timeliness during travel
Promote independence in daily activities yet be mindful of client’s intellectual and physical disabilities and assist with meals, cooking, household maintenance, grooming and dressing, as assigned
Assist with training employees to perform their direct support duties in accordance with the policies and procedures of Adjoin
Monitor and report to case responsible person on client and employee activities
Complete CSPs, monthly/quarterly reports, evaluations, intakes and assessments under the direction of the case responsible person
Document required activity records, reports, incidents, and client evaluations in a timely, complete, and accurate manner as required
Address challenging behaviors proactively and respectfully, using methods outlined in plans and training
Implement individual training and support programs under the direction of the case responsible person
Review activity logs of clients and report significant events or concerns to case responsible person
Participate in the evaluation of the client through completion of task analysis and other requested assessment devices
Qualification
Required
High School Diploma or Equivalent
18 years of age or older
Aptitude for confidentiality, organizational skills and good interpersonal skills
Certification in an approved First Aid and CPR class; or successful completion within 45 days of the date of hire
Successful completion of pre-employment drug screen and background check
Insured, registered vehicle in safe working condition, and proof of current CA driver's license
Acceptable DMV record
Ability to travel within and throughout the counties in which services are provided a minimum of 75% of the time
Preferred
Bachelor's degree or 3 years equivalent experience and/or educational equivalent in working with individuals with disabilities
