Responsibilities
Assess patient medical, psychosocial, and care transition needs
Collaborate with physicians and care teams to determine the most appropriate discharge plan and care goals
Evaluate community, insurance, and support resources required for ongoing care
Monitor patient care progression to ensure timely and efficient treatment and discharge
Identify and resolve barriers to discharge, escalating when necessary for resolution
Participate in interdisciplinary rounds to align discharge goals and expected timelines
Develop individualized discharge plans in collaboration with patients, families, and the care team
Initiate referrals for post-acute services, including home health, rehab, DME, or skilled nursing placement
Ensure patient and family understanding of discharge instructions and follow-up care needs
Document assessments, plans, interventions, and patient/family interactions accurately in the medical record
Communicate effectively with payer sources, utilization review teams, and community resources
Track and report avoidable delays or barriers impacting care progression
Support new team members through orientation and mentorship when needed
Participate in departmental performance improvement and professional development activities
Maintain required competencies and education
Qualification
Required
BSN
BLS
experience as a Registered Nurse
Associate Degree in Nursing or Diploma in Nursing
Active Registered Nurse license (State or Multi-State Compact)
Basic Life Support (BLS) Certification
Minimum 3 years of acute care bedside RN experience
Minimum 2 years of hospital-based case management or payer case management experience
Strong communication, critical thinking, problem-solving, and coordination skills
Preferred
Bachelor's Degree in Nursing
ACM or CCM Certification
