The Support Coordinator Trainee will participate in training and field experience to develop the skills necessary for ID/DD Support Coordinators, focusing on assessment, planning, and coordination of services for individuals with developmental disabilities.

Responsibilities

Completes initial and annual assessment of individuals for planning services and supports

Develops Individual Support Plan based on assessment information in collaboration with the individual and/or family

Provides linkage, coordination, and monitoring of services/supports to include Medicaid Waiver Services provided by private providers, other community agencies, natural support systems and other units in the NCSB

Maintains chart documentation that is in compliance with agency guidelines and state regulations. Ensures that documentation substantiates billing for services

Conducts face to face visits at a minimum of every 90 days and completes monthly activity related to the individual service plan

Monitors progress, service delivery, and individual satisfaction by providing monthly service to all individuals on assigned caseload. Provides increased frequency of service whenever clinically indicated

Identifies and accesses resources to meet identified needs and assist individuals in procuring and maintaining resources and entitlements

Assists individual/family in obtaining needed services on an emergency basis, i.e. hospitalization, housing, food, medications, etc
