Responsibilities
Conduct comprehensive assessments
Develop individualized recovery plans
Coordinate appropriate levels of care
Ensure services are delivered in compliance with contract standards
Provide community-based support through beneficiary visits
Document interactions and care provided
Collaborate with a broad network of community providers
Serve as a liaison between clients, providers, and internal teams
Represent the program in meetings
Assist with front-desk coverage and call screening as needed
Occasionally transport clients to appointments using a state vehicle
Qualification
Required
Bachelor degree
valid driver's license
experience in the mental health field is beneficial
