Case Manager (Day Program)
Social Vocational Services (SVS) has been serving adults with intellectual and developmental disabilities throughout California since 1977. They are seeking a full-time Case Manager to supervise and monitor clients during daily activities and develop individual client plans. The role requires effective communication and the ability to assess and chart client progress.

Responsibilities
Supervise and monitor clients during daily activities (including, but not limited to: paid work, social skills building, art, recreation, volunteering)
Develop & monitor individual client plans within the Program
Assist with personal hygiene needs as needed
Assess, evaluate, implement and chart client’s progress on goals and objectives
Qualification
Required
High school diploma, GED or equivalent
Good driving record and valid California driver's license
Post-job offer must obtain DOJ/FBI fingerprint clearance
Must pass post-job offer job-related physical exam including TB test
First Aid/CPR certification (SVS will provide)
Effective written and verbal communication
