Director, Virtual Care Program

Advanced Metabolic Care & Research (AMCR) is a private Endocrinology practice that has been delivering high quality endocrine care in San Diego for over 30 years. They are seeking a Director for their Virtual Diabetes Care Management Program to develop, launch, and lead a virtual care program aimed at improving patient outcomes and quality of life for individuals with diabetes.

Medical Practice

Responsibilities

Partner with practice leadership to develop and implement a comprehensive virtual care program, including: Establishing patient participation criteria & identifying eligible patients
Creating a patient referral process from practice clinicians to the virtual care program
Selecting and implementing clinical and financial software tools required to manage the program
Creating and selecting patient education materials, policies, and protocols that enhance the virtual education experience
Establishing measurable Key Performance Indicators (KPIs) for the program
Initiating virtual care activities
Grow and maintain a robust virtual care clinical practice
Deliver high quality billable care to eligible patients, including Remote Patient Monitoring, Principal Care Management, Chronic Care Management, Diabetes Self-Management Education and Support
Collaborate with AMCR’s clinical team to create a seamless continuity of care between traditional diabetes care and virtual care services
Develop and implement individualized care plans for diabetes management under the supervision of the practice clinical leadership
Utilize a variety of data sources, including CGMs, pumps, EMR, SDoH, etc. to track patient progress and adjust care plans as needed
Assist with CGM and pump starts and persistence
Help manage medications, including insulin and other diabetes-related therapies
Monitor and evaluate the effectiveness of treatment plans, working with practice clinicians to make adjustments as needed
Provide expert leadership and supervision to all virtual care program staff, in collaboration with practice owner and leadership team
Solicit input from the team in development, implementation and continuous improvement of the virtual care program
Provider training and mentoring related to virtual care program implementation
Work with the leadership team to establish and meet or exceed clinical outcomes, patient satisfaction, and staff satisfactions goals
Actively work to continuously improve quality and clinical outcomes
Ensure all virtual patient care meets established medical standards and best practices
Maintain accurate and thorough patient records, ensuring compliance with regulatory and billing requirements
Document patient progress and outcomes in electronic health records
Participate in and lead quality improvement initiatives to enhance patient care and virtual care operations
Stay current with advancements in diabetes care, virtual care and participate in continuing education opportunities
Serve as a key member of the practice operations team, participating in key operational, and growth aspects of the business
Participate in efforts to grow the practice’s virtual care services and the number of patients served by the program
Participate in recruiting and training additional CDCES staff as demand grows

Required

Minimum of 3 years of clinical experience delivering high quality, billable virtual care services to people living with Type 1 and Type 2 Diabetes
Successful experience measurably improving clinical outcomes and health status for people living with Type 1 and Type 2 Diabetes, as well as with helping people start and persist with using CGMs, pumps and other diabetes care management tools
Proficiency in using telehealth technologies, EHR systems, and diabetes management software and devices. Experience building a virtual care program a plus
A strong desire to improve care for people with diabetes and healthcare in general
Strong communication and interpersonal skills, with the ability to build rapport with patients and families
Exceptional organizational skills and attention to detail
Ability to work independently in a remote environment while maintaining strong collaboration with the interdisciplinary team
Active California license as a healthcare professional, e.g. RN, RD, PharmD
Certified Diabetes Care and Education Specialist (CDCES) certification required
