The Aortic Health Clinical Practice Coordinator plays a critical role in the multidisciplinary management of patients with aortic disease, providing clinical nursing expertise and coordination of care throughout all phases of treatment.

Responsibilities
Support the Aortic Health Program for the Heart & Vascular Center
Collaborate with providers across the cardiac surgery and vascular surgery teams
Provide clinical nursing expertise and coordination of care for patients with aortic disease throughout all phases of care
Manage from initial referral and diagnostic work-up through surgical or interventional treatment, post-operative recovery, and long-term surveillance
Provide expert clinical oversight, patient education, and care planning
Act as a consistent point of contact and advocate throughout the patient journey
Support program development initiatives, quality improvement, and community education to advance awareness and outcomes for patients with aortic aneurysms, dissections, and other complex aortic conditions
Qualification
Required
Graduated from an accredited nursing institution; Bachelor of Science in Nursing required
Minimum of three years of cardiovascular nursing experience required
Preferred
Procedural training and experience preferred
Certification in Nursing Specialty preferred
