Responsibilities

Provide primary care services to program participants

Lead and support group medical visits focused on the pillars of Lifestyle Medicine including nutrition, movement, sleep, and stress management

Collaborate closely with advanced practice clinicians, dietitians, health coaches, and social workers to deliver whole-person care

Oversee chronic disease prevention and management strategies for conditions such as diabetes, hypertension, obesity, and hyperlipidemia

Help design, refine, and scale clinical workflows that make care more accessible, sustainable, and patient-centered

Support implementation and optimization of digital and AI-enabled tools to track progress, streamline communication, and improve outcomes

Pilot new models of outreach, mobile care delivery, and community-based primary care

Work in a hybrid clinic with both in person and virtual clinical services

Serve as a trusted clinical presence at community events and enrollment activations

Provide real-time feedback on patient-facing tools, educational content, and engagement strategies

Contribute clinical insight to the design and deployment of AI-enabled tools that support clinical decision-making
