Required

Bachelor's degree in Nursing, Healthcare, or related field

Excellent interpersonal, written, verbal, and team-building skills

Keen knowledge of the Affordable Care Act and evidence-based service delivery models that encompass care coordination, disease management, informatics, wellness, quality improvement and ancillary clinical services
