Admissions and Documentation Coordinator

Wellspring Lutheran Services is a statewide nonprofit dedicated to serving seniors, families, and children throughout Michigan. They are currently seeking a full-time Admissions and Documentation Coordinator to support the Monroe Skilled Nursing and Rehab team by managing patient admissions and documentation processes.

Responsibilities

Daily communication with Centralized Admissions teams on admissions, discharges and bed availability
Communicates with campus team on pending admissions, ensuring patient room is ready and all equipment is available upon admission
Proactively communicates with patients and families on needs prior to patient arrival
Completes admission contract with new patients/responsible party
Conducts campus tours
Coordinate patient testimonies with marketing
Works with campus administrator to increase and promote a strong customer service culture
Attends daily stand up meetings, care conferences, weekly Medicare meeting monthly QAPI and triple check meetings
Completes any and all face to face MDS’s as required
Communicates with Centralized MDS team on any clinical changes with patients and residents
Submits documentation for insurance authorizations and appeals

Required

Associates Degree in Business, Marketing, or Nursing required
Strong clinical judgment, communication skills, and commitment to compassionate care

Preferred

Current Licensed Practical Nurse license issued by the State of Michigan Board of Nursing preferred
Minimum of 2 years experience in skilled nursing or a long-term care setting preferred
Minimum of 2 years of nursing experience preferred (home health, medical, surgical or critical care) preferred
