Ennoble Care is a mobile primary care, palliative care, and hospice service provider. They are seeking a full-time, experienced Clinical Care Coordinator specializing in Nephrology/Hemodialysis to support dialysis patients through care coordination and collaboration with clinicians and families.

Responsibilities

Coordinate care for dialysis patients using Ennoble Care’s electronic medical record system

Maintain regular patient contact to provide support, coordination, and follow-through on care plans

Communicate with clinicians regarding patient needs, changes in condition, and emergent concerns

Document patient and partner communications clearly, accurately, and in a timely manner

Collaborate with a multidisciplinary care team to support continuity of care

Follow documentation standards and escalation protocols

Contribute positively to team workflows and daily operations

Qualification

Nephrology/Hemodialysis experienceClinical skillsHealthcare terminology knowledgeMicrosoft Office proficiencyOrganization skillsCommunication skills

Required

Minimum one year of experience working with nephrology/hemodialysis patients

Comfortable communicating by phone and in person throughout the day

Strong organization skills and the ability to manage multiple patients and priorities

Knowledge of basic healthcare terminology, conditions, roles, and care principles

Clinical skills including obtaining vital signs and drawing laboratory studies

Proficiency with Microsoft Outlook and other Microsoft Office programs

Ability to work full-time, Monday through Friday (approximately 8:30am to 5:00pm CST), on-site, in Tulsa, Oklahoma

Valid, unencumbered driver's license, reliable personal vehicle, and current auto insurance, with the ability to provide proof (some travel between facilities may be required)

Ability to pass a background check and drug screen
