They are seeking a Case Specialist to provide comprehensive case management services, assess family needs, develop individualized service plans, and coordinate community resources.

Responsibilities
Conduct comprehensive family assessments to identify immediate and long-term needs
Conduct home visits, evaluate resources, and develop individualized family support plans
Provide ongoing case management, follow-up, and crisis intervention
Advocate for families and collaborate with multidisciplinary teams to promote positive outcomes
Transport families in personal vehicle
Qualification
Required
Bachelor's degree in social work, counseling, or a related field
Previous experience in case management or human services
Valid driver's license, auto insurance, and access to a reliable vehicle
Ability to pass all required background clearances
Ability to pass pre-employment, post-offer drug screening
Availability to work non-traditional hours, including evenings and weekends
Preferred
Strong understanding of family dynamics and related challenges
Excellent problem-solving, advocacy, and communication skills
Ability to work independently and collaboratively as part of a team
Strong organizational, time-management, and documentation skills
Proficiency in Microsoft Office and case management software
Ability to connect effectively with individuals from diverse backgrounds
