Responsibilities
Screen and assess patients to determine nutritional needs and status
Interpret clinical and laboratory data related to nutrition care
Provide nutrition education to patients and families, including food/drug interaction guidance
Document patient diet instructions and understanding in the medical record
Modify patient menus and consult with physicians as needed
Conduct follow-up care and ongoing nutritional evaluations
Qualification
Required
Minimum 1 year of experience required; 2 years preferred
Degree in Dietetics or Foods and Nutrition from an accredited institution
For acute care: Registration with the Commission on Dietetic Registration (CDR) required
For post-acute/long-term care: NY State Certification as 'Dietitian-Nutritionist' required at time of hire
