ACTIVITY COORDINATOR, BER - GPU

Responsibilities
Leads activity programs to meet a variety of interests as well as the physical, cognitive, and psychosocial needs of patients at SGMC Berrien Campus
Performs assessments of patients including social background, activity interests, abilities, physical limitations, and personal need for meaningful activity program
Maintains activities related documentation in accordance with CMS guidelines
Participate in staff IDT meetings and care plan meetings
Identify ways to accommodate patient preference and customary routines into their plan of care and daily activities
Attend mandatory in-services and maintain CPR certification
Willingly accepts and performs other assignments as delegated
Performs necessary activity related tasks at SGMC Berrien Campus as necessary
Qualification
Required

Performs assessments of patients including social background, activity interests, abilities, physical limitations, and personal need for meaningful activity program
Maintains activities related documentation in accordance with CMS guidelines

Identify ways to accommodate patient preference and customary routines into their plan of care and daily activities
Attend mandatory in-services and maintain CPR certification
Willingly accepts and performs other assignments as delegated
Performs necessary activity related tasks at SGMC Berrien Campus as necessary
Working knowledge of therapeutic recreation/activities practice
Necessary hours of training to maintain current certifications/licensure
CPR (ACLS, BTLS, NALS, PALS, etc.as required by job description)
Departmental Training Calendar compliance
Specific training for new or updated services/equipment relating to job requirements
Technical/Clinical skill competencies completed annually
Ability to recognize and report changes in patient condition
Works primarily in hospital setting, consistent shift and unit
Requires moderate to frequent lifting (excess of 50 lbs.), bending, and assisting in same, using correct techniques
Likely to be exposed to blood and body fluids and infectious diseases on a daily basis
Requires extensive walking during shift
Physical functional capacity for healthcare workers as defined by the Saunders Method (very heavy)
