They are currently seeking an experienced, clinically confident Home Health Registered Nurse (RN) – Level 2 to manage a consistent caseload while collaborating closely with physicians and interdisciplinary team members to ensure excellent patient outcomes.

Responsibilities
Perform comprehensive nursing assessments and manage ongoing home health care
Deliver skilled nursing services including medication management, wound care, chronic disease management, and patient education
Communicate changes in patient condition to Physicians, Case Managers, and the Director of Nursing
Maintain accurate, timely clinical documentation on company-issued devices
Coverage Area: Fixed, compact territory to minimize drive time
Follow all company policies, clinical standards, and regulatory requirements
Perform other duties as assigned
Qualification
Required
Current California RN License (required)
1–3 years of clinical nursing experience
Valid California Driver's License
Ability to travel throughout the San Mateo Peninsula and surrounding areas
Strong clinical judgment, organization, and communication skills
Preferred
