Amedisys Home Health is a compassionate care provider dedicated to patient and employee well-being. They are seeking an RN Case Manager for Hospice services to perform patient assessments, collaborate with care teams, and manage patient care plans while ensuring effective communication and support for patients and families.

Responsibilities

Performs patient assessments and collaborates with the care team to develop and implement a plan of care

Makes referrals to other hospice care disciplines as needed or documents rationale for not doing so

Develops, prepares, and maintains individualized patient care progress records with accuracy, timeliness and according to care center policies

Records pain/symptom management changes/outcomes as appropriate

Teaches patients, families, and caregivers appropriate patient care techniques

Regularly communicates with clinical manager and care team

Performs on-call responsibilities and on-call services to patients/families as assigned

Other duties as assigned

Qualification

Current RN licenseClinical RN experienceCPR certificationHospice experienceElectronic medical record (EMR)Manage multiple prioritiesCommunication skillsInterpersonal skillsWork independently

Required

Current RN license in the state(s) where you will work

Minimum 2 years of strong clinical RN experience in an acute healthcare setting (Required)

Current CPR certification

Preferred

Hospice or home health experience

Strong communication and interpersonal skills

Ability to work independently and manage multiple priorities

Comfortable with daily travel between facilities

Proficient in electronic medical record (EMR) systems
