Evaluates the need for medical social casework services to patients in their place of residence
Communicates patient needs to the physician and hospice interdisciplinary team
Plans quality medical social work services
Implements social work services to patient and their families in their place of residence
Evaluates medical social work services plan on an on-going basis
Maintains assessment and progress documentation accurately
Attends and presents appropriate patient-related information at the Interdisciplinary Team Meetings
Participates in the quality improvement program
Qualification
Required
Master of Social Work degree from a school accredited by the Council on Social Work Education
One year social work experience in a health care setting required
Current CPR certification
Preferred
Clinical experience with geriatric, or terminally ill populations desirable
