Responsibilities

Accountable for professional social services, which encompasses the provision of case management services designed to strengthen, rehabilitate, and preserve individuals and/or families; and provide protection and care for children and vulnerable adults in need of supplementary, substitute care, or provide full-time intake services
Oversee ongoing community-based services to clients with significant disabilities
Responsible for planning and monitoring of accessibility needs and in-home personal care services to ensure clients remain as independent as possible in both the home and community-based settings
Function as lead workers and direct the work of lower-level employees
Serve as Support Coordinator (SC) for people enrolled in the Home and Community-Based ID/DD Waiver program
Manage and maintain caseload of people receiving ID/DD Waiver services
Develop and maintain a person-centered Plan of Services and Supports (PSS) for each person on caseload
Revise/update each person’s PSS at least annually or when changes in the person’s circumstances occur or when requests are made by the person/legal representative
Coordinate and monitor identified supports and services through regular monthly contact, face-to-face quarterly reviews, and annual assessment and re-certification meetings
Ensure services are adequate, appropriate, and meet the person’s needs
Travel throughout the six southern counties will be required
Other duties as assigned

Bachelor’s Degree in PsychologyIDDCase managementPerson-centered planningTeamwork

Required

Knowledge of client support best practices
Willingness to work as a team

Preferred

Typically requires a Bachelor's Degree and 4-7 years of experience
A Bachelor's Degree in Psychology or related mental health field
Four (4) to seven (7) years of experience
One (1) year of experience working with people with IDD
Must be eligible to drive a State of Mississippi vehicle
