Support Manager, Midshore

Responsibilities
Facilitate Community Supports for people with disabilities to live within their community, focusing on choice and self-determination
Manage and coach Direct Support Professionals (DSPs) to coordinate person-centered planning activities with the Coordinator of Community Services
Ensure compliance with applicable DDA and other regulations
Be a liaison between all stakeholders and Fello regarding services and compliance with the person-centered plan
Be a member of the Leadership Council
Qualification
Required
Experience working with people with disabilities
Providing case management and/or planning facilitation
Proficiency in MS Office Suite (Word, Excel, Outlook)
Ability to exercise good judgment in sensitive situations
Ability to work well under pressure in an ever-changing environment
Team player able to respond to changes and/or crisis in a calm and productive manner
Strong oral and written communications and interpersonal skills
Ability to manage positive communication in difficult situations
Ability to provide candid/constructive feedback
High school diploma
At least five years' experience working with people with disabilities
At least two years providing case management and/or planning facilitation
Valid Maryland Driver's License with no egregious violations
Preferred
Experience with iCM documentation
Bachelor's degree
Experience in person-centered thinking
