Support the Case Managers & Social Workers by carrying out utilization and discharge planning activities as assigned
Communicating clinical information to payers for hospital certification
Tracking educational requirements of patient’s/families & assuring they are scheduled and completed
Communicating referral information to post-hospital service providers
Running worksheets/rounds reports
Qualification
Required
High School Diploma with experience in the medical field
Preferred
Prefer graduate of an accredited nursing program
Prefer three years of utilization review experience
