Senior Social Worker/Peer Coordinator - Integrated Care

Responsibilities
Provides curbside consultations with medical providers and other members of the medical team about plan of care for patients and provides feedback in a timely manner. The incumbent must use a high level of skill in conducting structured functional assessments and treating the complicated behavioral health problems of Veterans with a wide range of mental health and medical diagnoses. Treatment modalities include brief individual or group psychotherapy for individuals and/or families with an emphasis on evidence-based treatments appropriate to the setting. Independently performs case finding functions and outreach, identifying and screening patients for psychosocial needs. Independently provides consultation services to other staff regarding the psychosocial needs of Veterans and the impact of psychosocial problems on health care and adherence to treatment plan. Complies with all CPRS documentation procedures, including timeliness, standards for documentation and proper patient check out procedures (e.g., entering diagnostic and procedure codes)
Communicates effectively, both orally and in writing, with people from varied backgrounds and has the ability to remain organized. Participates in scheduling, triaging, and monitoring the flow of patients. Contributes to interdisciplinary care planning conferences, and team huddles/meetings
Adheres to, supports, and contributes to the development of operational policies and procedures of the primary care clinic, Integrated Care, MHS and the Medical Center, especially as it pertains to Peers in PACT. Executes ongoing program evaluation and data-driven quality improvement of Peers in PACT to monitor the effectiveness, which is reported to the Integrated Care Program Manager. Facilitates liaisons with other healthcare providers and/or programs in order to effectively coordinate services. Tracks, collects, and reports relevant data on Peers in PACT local, VISN, and National reporting, as indicated. May be assigned other administrative tasks as well at the discretion of the Integrated Care Program Manager
Is aware of, monitors, and complies with national and local guidelines and performance measures, including timely health care screening and completion of clinical reminders for all patients
Provides oversight of the Peers in PACT program by implementing program development, evaluation, and supervision of Peer Specialists
Serves on committees, work groups, and task forces at the facility, VISN, and/or VA Central Office levels, especially in an effort to improve peers support programing and outcomes. Keeps supervisor appraised of problems and recommended solutions to problems encountered in the incumbent's area of responsibility

Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE)
Persons hired or reassigned to social worker positions in the GS-0185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level
Social workers must be proficient in spoken and written English in accordance with VA Handbook 5005, Part II, chapter 3, section A, paragraph 3j
The candidate must have at least two years of experience post advanced practice clinical licensure and should be in a specialized area of social work practice of which, one year must be equivalent to the GS-11 grade level
Senior social workers must be licensed or certified by a state at the advanced practice level which included an advanced generalist or clinical examination
Skill in a range of specialized interventions and treatment modalities used in specialty treatment programs or with special patient populations
Ability to incorporate complex multiple causation in differential diagnosis and treatment within approved clinical privileges or scope of practice
Knowledge in developing and implementing methods for measuring effectiveness of social work practice and services in the specialty area
Ability to provide specialized consultation to colleagues and students on the psychosocial treatment of patients in the service delivery area
Ability to expand clinical knowledge in the social work profession, and to write policies, procedures, and/or practice guidelines pertaining to the service delivery area
