Abound Health is seeking an Intake Coordinator to join our team supporting individuals and families entering Innovations Waiver and State-Funded Services. In this role, you will serve as the first point of contact for clients and referral sources, ensuring a smooth and efficient intake experience.

Responsibilities

Serve as the primary point of contact for referrals and intake inquiries within assigned counties

Collect, review, and coordinate intake, referral, and admission documentation

Communicate regularly with families, referral sources, and internal teams to expedite intake timelines

Coordinate with Program Directors and clinical teams to ensure smooth client transitions

Identify staffing needs and communicate with Staffing and Onboarding teams

Participate in intake-related meetings and advocate for clients and families

Ensure accurate documentation is uploaded and maintained in required systems

Required

Credentialed as a Qualified Professional (QP) under one of the following: Master's degree in a related human services field with 1 year of IDD experience, Bachelor's degree in a related human services field with 2 years of IDD experience, Bachelor's degree in an unrelated field with 4 years of IDD experience

Strong attention to detail and organizational skills

Excellent written and verbal communication skills

Ability to manage multiple priorities in a fast-paced environment

Collaborative, professional, and client-focused approach

Bachelor's or Master's degree as required for Qualified Professional credentialing
