Responsibilities

Assessing, planning, implementing, and coordinating all case management activities with members

Evaluating the medical needs of the member to facilitate the member's overall wellness

Managing a caseload that includes supportive and medically complex members

Developing a proactive course of action to address issues presented to enhance short and long-term outcomes

Determining appropriate services and supports due to member's health needs, including prior authorizations, coordination with PCP and skilled providers, condition management information, medication review, community resources and supports

Required

Bilingual: (English & Spanish)

2+ years of experience in Behavioral Health or a Long-Term Care setting

Must reside in or near Zip Code 33010, 33011, 33012, 33013, 33014, 33015, 33016, 33017, or 33018

Willing and able to travel 50-75% of their time to meet members face to face in Miami-Dade County and surrounding areas in Florida

Computer proficiency in Microsoft Word, Excel, Outlook required

Bachelor's degree required, preferably in Social Work or related field required

Preferred

Ability to multitask, prioritize and effectively adapt to a fast-paced changing environment

Effective communication skills, both verbal and written

Managed Care experience

Case Management and Discharge Planning experience
