Healthcare Project Manager - Performance Improvement

The Project Manager - Performance Specialist will lead and support population health initiatives and practice performance programs to enhance clinical quality and financial performance.

Responsibilities

Lead and manage multiple population health and performance improvement initiatives (e.g., Annual Wellness Visits, Transitional Care Management)
Analyze practice data to identify trends, risks, and opportunities for improved outcomes and revenue growth
Support the rollout and monitoring of the Performance Enhancement Series (PES) program for underperforming practices
Create dashboards, reports, and presentations to track progress and communicate results to leadership and providers
Coordinate provider and consultant communications, including performance feedback and improvement plans
Manage scheduling and documentation for Physician Advisor (PA) program meetings
Conduct quarterly performance reviews and action plans for participating practices
Educate providers and staff on population health initiatives, workflows, and quality expectations
Collaborate with cross-functional teams to improve processes, resolve issues, and ensure HIPAA compliance

Required

Bachelor's degree in Business Administration, Healthcare Administration, or related field
Strong organizational skills with high attention to detail and confidentiality
Knowledge of healthcare performance metrics, population health strategy, and value-based care
Proficiency in Microsoft Office (Excel, Word, PowerPoint); SQL or reporting tools a plus
Excellent communication and relationship-building skills with physicians, staff, and leadership

Preferred

Master's degree plus 3+ years of experience in healthcare project management, hospital operations, or clinical performance improvement
