Responsibilities
Coordinates the post-acute plan discharge arrangements including post-acute care and Durable Medical Equipment (DME) set up
Provides logistical support to ensure a safe and timely discharge for patients to include but is not limited to patient discharge disposition and arrangements, post-acute discharge planning, and coordination of discharge activities
Qualification
Required
LA LPN license
LA Social Work license
Medical Assistant Certification
Ability to read, understand, and follow through on written instructions
Knowledge of multiple software programs, including Microsoft Word, Excel, and Internet
Familiar with office machines and equipment, such as copier and fax machine
Able to read medical terminology and assemble results from medical record
Ability to demonstrate effective written and verbal communication skills
Possess emotional and professional maturity
Highly organized and efficient in establishing priorities and performing tasks within specific timeframes
Self-motivated requiring minimal supervision
Must possess good physical health
Lifting at least 10 pounds is required
Preferred
Bachelor's Degree or higher in Social Work
one year healthcare experience in an acute care setting
