Responsibilities
Provide skilled, patient-centered nursing care based on individualized care plans
Accurately document all visits, assessments, and treatments using electronic health records (EHR)
Communicate effectively with patients, families, and the home health care team
Ensure compliance with Arizona home health regulations and company policies
Help patients meet their health goals through compassionate and professional nursing
Qualification
Required
Current Arizona Licensed Vocational Nurse (LVN) license in good standing
Graduate of an accredited vocational nursing program
Minimum 1 year of nursing experience (home health experience preferred)
Physical ability to assist and move patients as needed
Strong communication and documentation skills
Proficiency with electronic charting and care coordination software
Valid driver's license, reliable transportation, and ability to travel to patient homes in Sun City and surrounding Maricopa areas
