Qualification
Required
Doctor of Medicine (M.D.) or Doctor of Osteopathy (D.O.) from an accredited school of medicine
Completion of an accredited Residency Program
Licensed or eligible for licensure in Indiana
Board-Certified or Board-Eligible
Active and unrestricted DEA license or ability to obtain prior to start
Commitment to the mission of Indiana Health Centers, Inc
Preferred
Prefer working at an FQHC, CHC, or PCMH
