The Kidney Hero Navigator is responsible for executing the Kidney Navigator program, providing education and support to patients requiring dialysis, and enhancing their overall experience during their care journey.

Responsibilities

Meets with patients requiring outpatient dialysis placement or for chronic kidney disease (CKD) education as defined by program

Educates patients on the progression of disease for chronic kidney disease (CKD), transplants, dialysis, modality options, access options, and insurance

Demonstrates empathy for individuals based on a thorough understanding of patients’ individual needs, enthusiastically presenting Strive’s care model

Provides a hand-off to Strive’s Enrolment Associates, sharing what was learned about the patient’s unique situation and ongoing needs through our care management platform to enable an excellent patient experience

Provides quality hand-off to departments post-discharge program if the patient chooses to not enroll in Strive’s care program

Required

2+ years combined of related education, experience, or certification

Efficient and reliable transportation allowing for the ability to commute to patient homes or hospitals

Provides in-person patient care which may include standing, sitting, walking, pushing, pulling, and lifting

Internet Connectivity - Min Speeds: 3.8Mbps/3.0Mbps (up/down): Latency <60 ms

Preferred

Licensed LPN/Social Worker/Dialysis Technician

1+ years experience in dialysis setting
