University of Michigan Health-Sparrow is seeking a Health Record Analyst I to ensure the completeness of medical records in accordance with established policies and standards. The role involves analyzing medical records, processing continuity of care requests, and providing quality copies of medical records while adhering to organizational guidelines.

Responsibilities

Analyze inpatient, outpatient and observation medical records for the health system for the presence and signature on the following key documents: discharge summary, history and physical, operative reports, cardiac catheterizations, consultations and procedure notes
Assist Physicians and other direct patient care professionals in answering questions regarding the Electronic Health Record medical record completion process
Report quality issues of scanned documents in EPIC to the HIM Operations Manager
Serves as coverage for continue care and transcription related phone calls
Process all accounts in assigned work queues within EPIC accordingly to the departmental and organizational policy and procedures
Perform final analysis on all inpatient, outpatient and observation records at the time of discharge
Perform all duties according to policy and procedures with 98% accuracy of physician deficiency assignment
Utilize Lead Health Record Analyst as a first contact for daily operations
Escalate issues or communication concerns with physicians to the HIM Operations Manager in a timely manner
Process incoming continuity of care requests from internal and external physician offices and/or other medical facilities that do not require patient authorization
Utilize all tools provided to process requests as timely as possible
Recognize all non-continued care requests requiring medical record authorization according to HIPAA policy and procedure; refer to the ROI Coordinators responsible for processing such requests
Reports all questionable requests to HIM Operations Manager responsible for the Release of Information Unit in a timely manner
Perform all duties according to policy and procedure with 98% accuracy rate
Serves as coverage for ROI Coordinators for front desk presence and telephone coverage
Provide copies of the medical record for disclosure of protected health information (PHI)
Ensure copied medical records adhere to the minimum necessary guidelines and organizational policies
Prioritize copies of medical records to meet set deadlines
Produce documents from EPIC on behalf of Release of Information
Order charts from outside record storage facility and organize by request and date received
Identify documents within the medical record that have been incorrectly placed in the chart (i.e. incorrect patient, mislabeled document, etc.)
Report issues to HIM Operations Manager
Perform all duties according to policy and procedures with 98% accuracy rate
Serves as coverage for ROI Coordinators for front desk presence and telephone coverage

Required

1 year of experience in Health Information Management or medical office setting
High school diploma or GED
Ability to multitask in high demand settings
Knowledge of medical record format and content for all records
Excellent communication skills (verbal and written communication)
Ability to interact with customers in a friendly service-oriented manner
Excellent problem-solving skills
Ability to work with minimum direct supervision
Demonstrates ability to use a keyboard as may be required to perform the essential duties of the job

Preferred

Medical Office Assistance
Experience with deficiency analysis in an acute healthcare setting
Experience with processing continuity of care requests
Experience with medical record copying
Knowledge of the components within a medical record
