Clinical Assessor (New Hampshire)

As a Clinical Assessor, you will conduct needs-based eligibility assessments for individuals applying for Medicaid-funded personal care services, ensuring appropriate placements and care planning to improve long-term health outcomes.

Responsibilities

Conduct person-centered assessments by reviewing relevant records and consulting collateral sources to ensure accurate evaluations and well-informed recommendations for appropriate levels of care and support services
Complete assessments promptly and within the scope of practice, meeting all contractual and regulatory requirements
Recommend appropriate levels of care and services based on comprehensive evaluation of the patient’s physical, behavioral, and psychological functioning
Leverage clinical expertise and knowledge of psychiatric disorders as outlined in the latest Diagnostic and Statistical Manual (DSM) applicable in New Hampshire
Apply knowledge of mental health standards of practice, as well as intellectual and developmental disabilities, geriatrics, and conditions commonly requiring long-term care services and supports
Utilize the state’s electronic system and proprietary platforms to efficiently manage document, and track assessment activities
Adhere to program workflows and ensure compliance with established processes and standards
Understand and adhere to all corporate policies, including those related to HIPAA and its Privacy and Security Rules

Required

Must possess an active, unrestricted RN license with eligibility to practice in the state of New Hampshire
Graduation from an accredited college or university with a degree in a relevant field, along with recent hands-on experience conducting clinical assessments, ensuring accurate evaluations, and providing appropriate next level of care recommendations
Proficiency in medical record abstracting, including a strong understanding of medical record organization, medical terminology, and disease processes
Detail-oriented with excellent organizational skills and the ability to prioritize and complete tasks efficiently within established timeline
Demonstrate strong typing and computer skills, including proficiency in Microsoft Office applications (Word, Excel, Outlook, and Teams)

Preferred

Knowledge of public mental health systems and medical programs is preferred
