Responsibilities
Coordinate discharge planning and ongoing care for patients under psychiatric medication management
Schedule follow-up appointments with outpatient psychiatric providers and therapists
Support patient access to prescriptions, transportation, insurance coverage, and social services
Communicate with pharmacies, families, guardians, and care teams to ensure continuity of care
Document case management activities in the Electronic Health Record (EHR) system
Participate in interdisciplinary team meetings and contribute to holistic care planning
Qualification
Required
Associate's or Bachelor's degree in healthcare, behavioral health, human services, or related field preferred
1–2 years of experience in case management, care coordination, or behavioral health services
Level 1 Fingerprint Clearance Card (or ability to obtain upon hire
Strong communication, time management, and organizational skills
Familiarity with EHR systems and HIPAA compliance
Preferred
Bilingual (English/Spanish) is a plus
