Ursamin is rebuilding the backend that will power clinical decisioning across health systems. As the first full-time engineer, you'll own the backend foundation, implementing a FHIR-based core model and API Gateway while ensuring PHI governance and collaborating with clinical stakeholders.

Responsibilities

Lead design and implementation of the system of record and API Gateway as the single front door for all data

Define and implement a focused FHIR subset suitable for production and design normalization patterns to ingest and reconcile data from health exchanges, partner sources and direct EHRs (starting with athenahealth)

Build reliable, event-driven ingestion pipelines with replay, deduplication and provenance metadata for documents

Deliver CI/CD, automated deployments, monitoring, and incident management

Implement field-level encryption, access controls and HIPAA-aligned patterns for PHI governance

Collaborate with clinical and product stakeholders to translate workflows into resilient technical designs

Mentor contractors and future hires, produce high-quality documentation, and ensure handoffs are production-ready

Required

Six or more years of backend engineering experience with at least three years building backend and platform systems

Strong design knowledge of REST/GraphQL APIs and API Gateway patterns (Azure API Management, Kong, Apigee)

Deep experience with event-driven systems such as Kafka or Azure Event Grid and practical experience integrating healthcare systems (FHIR/HL7 or equivalent)

Demonstrated knowledge of PHI governance, encryption strategies and secure architecture

Experience with CI/CD, infra-as-code tools (Terraform/Bicep), cloud deployments (Azure preferred), and observability tooling (OpenTelemetry, Prometheus, Grafana)

Excellent communication skills and the ability to convert clinical and product requirements into engineering deliverables
