Care Coordination Education and Training Supervisor

Responsibilities
Develops, revises, and administers education and training curriculums
Monitors St. Mary’s Healthcare System, NYS DOH and partnering health home policies and procedures to ensure training compliance and provision of optimal care coordination services
Screens candidates and participates in interviews
Prepares all necessary training documents and monitors completion of additional mandatory program trainings
Collaborates with supervisor/staff and provides feedback as well as intervention to ensure performance of new hires caseload/workload meets the expectation of the program
Works collaboratively with Quality and management team to identify staff skill building education and training needs
Organizes and provides ongoing education and training to address identified needs
Oversees and tracks care coordinators training and educational requirements and submits as needed to partnering health homes
Participates in all Health Home and care coordination team meetings
Maintains collaborative relationships with all internal personnel and outside agencies to ensure ongoing quality care coordination
Shadows care coordinators to initial home visits and assists with case load coverage as needed
Participates in quality improvement activities and ensures incidents and complaints are reported and meet program regulatory requirements
Curriculum development transferable to structing a clear and persuasive grant narrative or white paper
Use of the educational sectors as leverage for professional connections and assists with staff’s overall development
Qualification
Required
Bachelor's degree with a minimum of 3 years' experience of care coordination duties, a lead in supervising and/or training staff
Master's degree with a minimum of 2 years' experience of care coordination duties, a lead in supervising and/or training staff
