Responsibilities
Oversee and administer the Resident Assessment process according to applicable policies and regulations
Facilitate the completion and updating of resident Care Plans within MDS and the IDT to reflect the resident’s most current needs
Coordinate skilled coverage for residents with managed care
Review Medicare Part A coverage and necessary documentation for eligible residents
Obtain signed physician certification forms as indicated
Participate in quality improvement initiatives as directed
Provide necessary documentation of all activities to meet regulations
Support various other Nursing responsibilities as needed
Qualification
Required
Licensed as a Registered Nurse in the State of Minnesota and must maintain licensure
One or more years' nursing experience in a long-term care setting required
Preferred
One year of MDS and care plan development experience preferred
CPR Certification preferred
