RPM Team Lead
TapestryHealth is focused on improving patient health outcomes through their RPM program. The RPM Team Lead will oversee daily operations of the Notification Center, ensuring effective communication and collaboration among team members to support patient navigation and management.
Responsibilities
Serve as initial point of contact for questions and serve as a subject matter expert to the team
Lead the onboarding process for new hires, ensuring a smooth integration into the team
Ensure protocols related to alerts are adhered to as established
Collaborate on refining and maintaining training materials to ensure clarity, accuracy, and consistency
Provide ongoing mentorship and support to foster individual confidence and team success
Partner with the other Team Lead’s to maintain consistent coverage and support when one is out
Identifies medical changes or abnormalities through monitoring for escalation and clinical review per protocol
Communicates with other Team Leads, Supervisor, and Manager regarding quality, system capacity, and resource requirements, facility and/or provider feedback (if applicable)
When not onboarding, maintain approximately 50% of a regular assignment to stay actively engaged in daily workflows
Foster a supportive team culture that promotes open communication, mutual respect, and shared success
Adhere to all established policy/protocol of the department and workflow
Works with Tapestry IT and Data Scientists to resolve discrepancies with patient data information
Protects patient information and adheres to the standard of confidentiality
Contributes and participates in continuous quality improvement activities

Qualification
Required
Clinical Degree (LPN/RN, Health Science, Medical Assistant)
Minimum of 1-year intermediate health care data analytics skills, medical chart audit, and/or clinical documentation review
Minimum of 1 of year hands on patient care experience
Proficiency in navigating electronic health records
Experience in performance management with a high level of comfort delivering feedback, and ongoing performance management audits
Proficient with Microsoft Office Suite
Ability to understand and interpret basic math logic such as averages and graphs
Preferred
Strong verbal/written communication
Ability to organize and prioritize tasks
Ability to work collaboratively, be attentive to detail and maintain a positive attitude
Skilled in conflict management and resolution
Familiarity with HIPAA regulations and patient privacy standards
