Mid LevelGuideStar Eldercare is a provider of neurobehavioral health services for residents in long-term care facilities. As a Clinical Therapist, you will offer evaluation and therapy while coordinating care plans with interdisciplinary teams to support elderly individuals with neurocognitive disorders and other psychiatric conditions.

Responsibilities
Provide evaluation
Provide condition-specific support therapy, or when indicated, insight-based therapy
Coordinate the patient’s care plan with the facility, PCP and GuideStar colleagues, and engage with the family as indicated
Qualification
Required
Ability to work as part of an interdisciplinary treatment team
Good time management skills
Able to function autonomously with regional Area Manager support
Master's degree in Mental Health Counseling or in Social Work
Currently licensed as an LMHC or LCSW in the state in which you are applying
Preferred
Passion for geriatrics
Experience in inpatient settings such as extended care facilities
Experience with brief bedside assessment screens (cognitive, mood, etc.)
Motivated by productivity-based positions
Ability to navigate and document on an Electronic Medical Record (EMR) system
