Quality Improvement Project Coordinator- Part Time Temporary

The QI Project Coordinator will work under the direction of the Quality Improvement Director to maintain and improve the quality and performance of clinical services through comprehensive coordination of assigned Quality Improvement Projects.

Responsibilities
Maintains a deep understanding of assigned projects (e.g. PCMH, Peer Review, PQI, Patient Satisfaction, Health Awareness Campaigns, Partnership Quality Incentive Program)
Collaborates effectively with the QI Team to assure project goals are progressing as planned and necessary adjustments communicated and made as needed in a timely manner
Assists the QI Project Manager with the peer review and patient satisfaction reporting processes by coordinating reviews, sharing service recovery opportunities, and supporting data analysis as needed
Assists the QI Manager and Quality Improvement Coordinators in conducting panel management of patients and outreach to patients who are due for a variety of screenings, appointments and immunizations
Assists with the Patient Centered Medical Home renewal process, including; documentation of workflows, ensuring collaboration with key stakeholders and assisting with advancing project goals and tasks
Assists the QI Director and QI Manager with a variety of short-term projects including; awareness campaigns, outreach campaigns, data validation and data entry for program compliance
Assists with drafting and disseminating updates with the organization on behalf of the department through emails, newsletters and other avenues, tailoring information to different audiences and groups
Assists in preparing materials for meetings, including summaries, reports, and supporting documentation
Assists the QI Project Manager with the PQI Process by tracking cases, updating data, and following up on pending requests to support timely review and closure
Performs other duties as assigned
Qualification
Required
High School Diploma required
Minimum three (3) year administrative experience
Minimum three (3) year working in an Outpatient Primary Care Community Health Center setting or equivalent experience strongly preferred (required if no healthcare-specific education or degree)
strong working knowledge of Microsoft Office Suite (including Excel)
experience with electronic medical records systems (preferably OCHIN Epic)
Preferred
bachelor's degree or some college coursework, especially in a health-related field
Knowledge of and experience with any or all the following: Quality Improvement Methodologies (IHI Model for Improvement, Lean, Six Sigma, etc.)
