This role emphasizes a 100% outpatient primary care model with a small patient panel and a strong support team, ensuring a balanced work-life environment.

Responsibilities
100% outpatient primary care
Value-based care model
Average of 18 patients per day
Small patient panel capped at approximately 450 patients
Dedicated care team including Medical Assistant and Scheduler
Administrative time built into the schedule
No nights, weekends, or call
Monday–Friday schedule
Clinic hours: 7:30 AM – 5:00 PM (last patient scheduled at 4:15 PM)
Qualification
Required
Residency trained in Internal Medicine or Family Medicine REQUIRED
Board Certified in Internal Medicine, Family Medicine, or Geriatric Medicine REQUIRED
Active and unrestricted medical license required (any U.S. state)
Commitment to value-based care and senior-focused medicine
Ability to manage a chronic care population in an outpatient setting
Spanish language skills required
